What is Xenical?

XENICAL is a weight loss agent called a lipase inhibitor. It is a potent, specific and long-acting inhibitor of gastrointestinal lipases. The deactivated enzyme is therefore not available to hydrolyse dietary fat, and this allows about 30% of the fat eaten in your meal to pass through the intestines in undigested form. As a result, your body cannot use this dietary fat as a source of energy or convert it to fat tissue. Xenical is not appropriate for everyone

Do not take XENICAL

  • if you are allergic (hypersensitive) to orlistat or any of the other ingredients of XENICAL (a full list is set out below);
  • if you have chronic malabsorption syndrome (insufficient absorption of nutrients from alimentary tract); or
  • if you have cholestasis (liver disorder, including problems with your gall bladder).

Take special care with XENICAL

Weight loss may also affect the dose of medicines taken for other conditions (e.g. high cholesterol or diabetes). You must discuss these and any other drugs that you may be taking with your doctor. Losing weight may mean your doctor will need to make adjustments to the dose of these medications. Xenical can cause harmless and temporary changes in your bowel habits, such as fatty or oily stools, due to the elimination of undigested fat in your faeces. The probability of this happening will increase if you diet is high in fat. Your daily intake of fat should be distributed evenly over three main meals because if Xenical is taken with a meal very high in fat, the possibility of gastrointestinal effects will be increased.

Pregnancy and Breast-feeding

Do not take Xenical if you are pregnant.

You should not breast-feed during treatment with Xenical as it is not known whether Xenical passes into human milk but there is a high likelihood that it does and this would be very harmful to the baby.

Ability to drive and operate machinery

Xenical does not affect your ability to drive a car or operate machinery.

Compatibility with other medicines

You must inform the doctor if you are taking or have recently taken any other medicines, even those not prescribed. If you are prescribed other medications or you buy medications from a pharmacy after we have issued you with a prescription for Xenical, you must inform your doctor or pharmacist that you are taking Xenical so that they are in possession of the full facts before another medication is recommended.

This is important as using more than one medicine at the same time can strengthen or weaken the effects of the medicines.

Xenical may affect the following medications:

  • Anticoagulant drugs (e.g. warfarin). Your doctor may need to monitor your blood coagulation if you are taking Xenical.
  • Ciclosporin. Co-administration with ciclosporin is not recommended.
  • Amiodarone is not recommended with Xenical.

It may be dangerous to take Xenical with the above or any other medication for that matter, so it is vital that full disclosure of your medical history is divulged to the doctor who is being consulted about Xenical.

Xenical will have an impact on the absorption of the fat soluble vitamins: A; D; E and K; and also that of beta carotene. You should therefore follow your doctor's advice in taking a well balanced diet rich in fruit and vegetables. Your doctor may suggest you take a multivitamin supplement. The vitamin supplement should not be taken at the same time as Xenical and we would recommend leaving a gap of 2 hours either side of Xenical and taking the vitamin supplement.

How to take Xenical

Always take Xenical exactly as instructed by your doctor. The usual dose of Xenical is one 120 mg capsule taken with each of the three main meals per day. It can be taken immediately before, during a meal or up to one hour after a meal.

Xenical should be taken with a well-balanced, calorie controlled diet that is rich in fruit and vegetables and contains an average of 30% of the calories from fat. Your daily intake of fat, carbohydrate and protein should be distributed over three meals. This means you will usually take one capsule at breakfast time, one capsule at lunch time and one capsule at dinner time. To gain optimal benefit, avoid the intake of food containing fat between meals, these include biscuits, chocolate and savoury snacks.

Xenical only works in the presence of dietary fat. Therefore, if you miss a main meal or if you have a meal containing no fat, you do not need to take Xenical.

If you forget to take XENICAL

If you forget to take your medicine at any time, take it as soon as you remember provided this is within one hour of your last meal, then continue to take it at the usual times. Do not change the prescribed dose yourself unless your doctor tells you to.

Potential Side Effects of Xenical

Like all medicines, XENICAL can cause side effects, although not everybody gets them.

The most common side effects with Xenical occur as a result of the localised action of the drug in the intestinal tract. It is possible that you will experience increased bowel activity with orange or yellow oily discharge. This results from the passage of undigested fat through your gut. These sorts of side effects are usually moderate in nature and can be influenced by a change in diet. Most patients who experience these side effects at the beginning of treatment notice that they subside with time. Very few patients feel compelled to cease the treatment because of side effects that they suffer.

Rare cases of allergic reactions have occurred with Xenical. The main symptoms are itching, rash, severe difficulty in breathing, nausea, vomiting and feeling unwell.

Other very rare side effects have included diverticulitis, gall stones and raised liver enzymes. Mild rectal bleeding has been reported in extremely rare cases.

In clinical trials some patients with type 2 diabetes experienced low blood sugar levels and bloating.

The use of an additional contraceptive method is recommended to prevent possible failure of oral contraception that could occur in case of severe diarrhoea.

You must contact your prescribing doctor immediately if you notice any of these or any other side effects while on Xenical.

Other ingredients

The other ingredients are microcrystalline cellulose, sodium starch glycollate, povidone, sodium lauryl sulphate and talc. The capsule shell consists of gelatine, indigo carmine (E132), titanium dioxide (E171) and edible printing ink.
