What is Premarin?

Premarin is a female hormone replacement therapy that comprises conjugated oestrogens (oestradiol).

Symptoms of the menopause result from the slow reduction in oestrogen secreted by the ovaries in women from about the age of 45 years. Symptoms include sudden hot flushes (particularly of the face, neck, and chest), dryness/itching of the vagina, and changing moods. Aging women are also at risk of brittle bone disease (osteoporosis) and those at particular risk of fracture can be protected by supplementary oestrogen.

How to use Premarin

Premarin comes as a tablet that is available in three strengths (0.3 mg, 0.625 mg, 1.25 mg). Your doctor will select the dose that matches your symptoms and their severity.

When you start taking Premarin will depend on whether you are already using another HRT. If you are not using another HRT, start on any convenient day. If you are using another HRT and you do not have a bleed each month, also start when convenient. If you are using another HRT and you have a bleed each month, start Premarin when you have completed the previous HRT pack. Take one tablet every day and, as soon as you finish one pack, start a new pack. Take the tablet whole with water, and at the same time every day. It is not serious if you forget or take more than one tablet. Forget a tablet – then take the tablet when you remember, and continue as you have been doing; take too many tablets – then you may feel sick, and/or you may have abdominal pain, tender breasts, giddiness, sleepiness, and vaginal bleeding.

Generally, Premarin is for a women who have had hysterectomy and do not have a womb. Such women will not usually have monthly bleeds. However, women who do have a womb may also be given another female hormone tablet called progesterone. These women will usually have monthly bleeds, which will reduce gradually over time until they stop. When discussing this medicine, your/your family's medical history must be disclosed to the doctor, who will want to see you regularly for check-ups.

Who can use Premarin?

You can take Premarin if you are post-menopausal and have life-disrupting menopausal symptoms or osteoporosis.

If you have had or have certain diseases, then you must not take Premarin as your HRT. Such diseases are listed as allergies to conjugated oestrogens or other Premarin ingredients, breast cancer, cancer sensitive to oestrogens (e.g., endometrium), an excessively thick womb lining, unexplained vaginal bleeding, thrombosis (blood clots in veins), heart attack or stroke (due to blood clots in arteries), other disorders of blood clotting, porphyria, and liver disease. If you have other diseases, then you must be careful when taking Premarin. These include growth of fibroids in your womb, growth of endometrial (womb) tissue outside your womb, previously excessive growth of endometrial lining, and an increase in risk of getting blood clots or cancer sensitive to oestrogens. Additionally, care is also taken when you have a disease of the liver, kidney, heart, ear (otosclerosis), immune system (systemic lupus erythematosus), or gallbladder (and gallstones); or headaches or migraine, diabetes, high blood triglyceride (fat) level, low blood calcium concentration, fluid retention, high blood pressure, epilepsy, or asthma. The doctor must be notified about all of these conditions as they can become worse or reappear. If you have to have surgery, then contact your doctor as soon as it is confirmed.

Mention all your medicines to your doctor as Premarin could affect the way that they work and vice versa. Some medicines can upset your regular bleeding cycle so mention all anticonvulsives, anti-infectives, St John's Wort-containing remedies, and metyrapone that you currently take. Tell those who take blood samples from you that you are on Premarin as your test results may not be as expected.

Premarin Side Effects

Apart from the diseases listed above that may return or appear or worsen, HRT can cause uterine bleeding vaginal inflammation/discharge/thrush, breast pain/tenderness/swelling, nipple discharge, changes in libido, body weight, hair growth, and mood (irritability, anxiety, and depression), feeling/being sick/bloated, abdominal pain, dizziness, swelling of parts of the face and body, skin problems, and joint pain/leg cramps.

You should be aware that women who use HRT report some diseases more frequently compared to women who do not use HRT. These are blood clots in veins (legs and lungs), heart disease, and stroke; breast, ovarian and womb (endometrium) cancer; womb thickening, and memory loss (when you first start Premarin at 65+ years of age). These severe side effects are very rare.
